Case report on Replantation of avulsed tooth

» CASE HISTORY
Six years back patient visited the hospital with fresh accidental injury with an empty socket corresponding to 21 tooth.
Patient told us that the tooth was lost accidentally in the same bus in which she sustained trauma. Patient was explained the modern technique of successful re-plantation of such teeth if preserved immediately in normal saline.
Patient traced the bus and fortunately retrieved the tooth well preserved and with in hours. which was then thoroughly rinsed with normal saline to clean the debris, treated with antibiotic (Gentamycin) and replanted in the socket after subjecting to in vitro RCT, and splinting was done. There was even Elli`s class 2 fracture which was built up & restored by composite filling. Patient was given a oral course of antibiotic(Cephadroxyl) for 5 days, advised semi liquid diet for a fortnight and strict oral hygiene using Hexidrine mouth wash for rinsing.
» Follow ups
There was some pain & irritation due to active inflammatory repairs but no secondary bleeding and/or infection. And after 2 months splint wire was removed.
Now after 6 years patient came for general dental check up and evaluation of her replanted tooth.
During the visit after the series of radio-graphs we noticed that the tooth which was replanted was healthy and intact with no bone resorption as you can see in the underlying radiographic picture

We noticed that vertical dimension of the tooth we treated (21) was not increased and there was clear opacities of perfect RCT done with ankylosis fusion of the tooth with socket as compared to the adjacent normal tooth(11).
We felt satisfied for we retained patient`s smile and confidence and above all, fully functional tooth even after six years.

Along with our excellent expertise, the credit goes to the patient for her, enthusiastic retrieval of the avulsed tooth from a bus, in the Punjab roadways bus depot well preserved, and in time. Credit goes to her body which actively accepted the replanted tooth with a neat repairing. Its worth here to evaluate the procedures for a carious tooth with exposure, REPLANTATION AFTER IN VITRO RCT compared to STANDARD RCT procedures, as MANY A POSTERIOR TOOTH WITH DISTORTED HOOKED BENT ROOT CANALS RESULT IN FAILURE RCT, and after the standard RCT the failed tooth also become unfit for replantation.
